Gene therapy is a category of treatments that work by modifying, replacing, or adding genes to a patient's cells to treat or cure a disease at the genetic level. Some gene therapies are one-time treatments intended to be curative — Zolgensma for spinal muscular atrophy, Casgevy and Lyfgenia for sickle cell disease, Hemgenix for hemophilia B. Others are ongoing therapies. The FDA has approved dozens of gene and cell therapies and has hundreds more in clinical trials.

The pricing is unlike anything else in medicine. Zolgensma launched at $2.1 million per one-time treatment. Casgevy runs $2.2 million, Lyfgenia $3.1 million. Hemgenix hit the market at $3.5 million. For a self-funded employer, a single gene therapy claim can exceed the entire specific stop-loss deductible in a single dose. The clinical case is often strong — a one-time cure that eliminates decades of chronic treatment cost — but the financing challenge is real. Manufacturers have experimented with outcomes-based warranties (partial refund if the therapy doesn't work) and installment-payment structures to spread cost over years. Stop-loss carriers now often require specific gene-therapy riders and disclosure of at-risk members.

The takeaway: if you're a self-funded employer with any member who could be eligible for a gene therapy (sickle cell, hemophilia, certain rare pediatric conditions), talk to your stop-loss carrier about coverage terms today. Gene therapy risk requires explicit contract language, not assumptions.
